This Notice describes how your health information may be used and disclosed and how you may access this information.

Our Commitment to Your Privacy

Campsen Wellness is required by law to protect the privacy of your protected health information (PHI), including medical records, billing information, and electronic communications, and to provide this Notice in compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). We use and disclose your PHI only as permitted by law, limit disclosures to the minimum necessary, and require our business associates to safeguard your information.

Uses and Disclosures without Authorization

We may use or disclose your health information without written authorization for the following purposes:

Treatment, Payment, and Health Care Operations

  • Providing, coordinating, or managing your physical therapy care

  • Billing, collections, and insurance-related activities

  • Clinic operations, quality improvement, training, and compliance

Care Coordination

  • With referring or consulting providers involved in your care

  • To obtain authorizations or approvals for services

Workers’ Compensation / Occupational Injuries

  • As required by Iowa law, information related to work-related injuries may be shared with employers and their representatives.

  • Only information relevant to the injury will be disclosed.

Public Health, Safety, and Legal Requirements

  • To public health or oversight agencies as required by law

  • In response to court orders, subpoenas, or administrative proceedings

  • To law enforcement officials when required by law

  • To prevent or reduce a serious threat to health or safety

  • For military, national security, or correctional purposes as authorized by law

Family and Caregivers

  • To family members, personal representatives, or others involved in your care when appropriate.

  •  In emergencies, only information necessary for your care will be shared.

Other Permitted Uses

Appointment reminders and practice-related communications. We do not sell or share your information for marketing purposes.

Authorization and Client Rights

Uses or disclosures not described in this Notice require your written authorization, which may be revoked at any time in writing. You have the following rights regarding your PHI:

  • Access: You may inspect or obtain copies of your health records, with limited exceptions permitted by law. Reasonable, cost-based fees may apply.

  • Amendment: You may request that we correct or add information to your records if you believe they are inaccurate or incomplete. If we deny your request, you may submit a written statement of disagreement for inclusion in your record.

  • Accounting of Disclosures: You may request a list of certain disclosures of your health information by our clinic or our business associates for up to six years prior to your request, as permitted by law.

  • Restrictions: You may request restrictions on certain uses or disclosures of your PHI. We are not required to agree to most requests; however, we must comply with a request to restrict disclosure to a health plan if you pay for a service in full out-of-pocket.

  • Confidential Communications: You may request that we communicate with you in a specific way or at a specific location (for example, by mailing records to a different address).

  • Paper Copy: You may receive a paper copy of this Notice upon request.

  • Breach Notification: You have the right to be notified if a breach occurs that compromises the privacy or security of your PHI.

Billing & Collections

If your account is referred to a collection agency, diagnostic details will not be disclosed. Only limited information necessary to identify charges may be shared, as permitted by law.

Changes to this Notice

Campsen Wellness reserves the right to revise this Notice at any time. Any changes will apply to all health information we maintain. Revised Notices will be available upon request, posted in our clinic, and made available on our website.

You may file a complaint if you believe your privacy rights have been violated. You will not be retaliated against for filing a complaint. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR).
